CREATIVE CONSENT

This Agreement is entered into by and between Mosa Pet Spa & Boutique and the undersigned individual or entity (the "Client") as of the date of signing. This Consent of Care form acknowledges that the Client has reviewed and agrees to the policies established by the Business. These policies are designed to promote a clear understanding of expectations and ensure a positive relationship between the Business, the Client, and the pets under the Business’s care. The well-being and comfort of your pet(s) remain the primary focus of all services provided.

Medical Release

The safety and well-being of every pet in our care is our top priority. While Mosa Pet Spa & Resort LLC exclusively uses natural and pet-safe products, we acknowledge that individual pets may have unique sensitivities. As a result, we cannot guarantee that any product is completely hypoallergenic and are not liable for allergic reactions or aggravation of pre-existing conditions. To prevent injury a small patch test will be applied and assessed prior to Full coloring Requested.

Additionally, Mosa Pet Spa & Resort LLC is not responsible for any underlying medical conditions (such as hotspots, skin growth irritation, heart disease, respiratory disease, arthritis, or clotting disorders) or any potential aggravation of these conditions that may occur as a result of the creative application process. By proceeding with our services, you acknowledge and accept these terms.

Color Variation Release

The client acknowledges that coloring products may produce varying results depending on their pet’s coat type, and the final shade and vibrancy may differ from expectations.

Additionally, the client understands that the longevity of the color depends on the type of product used, including airbrush, chalk, semi-permanent, or permanent dyes. Some colors may fade gradually over time, while others may only be removed by trimming the dyed fur. The client has discussed the permanency of the chosen product with their groomer and accepts the results and maintenance requirements associated with their selection.
